Use this form if you want to schedule time for a series program (weekly, biweekly, monthly, or daily.) This form must be accompanied with a signed Program Agreement, a video of your first episode, proof of residence, and valid photo ID. All materials, including the videotape, must be received by the open application deadline date to be considered for the upcoming quarter.

The Program Agreement is required of all producers who submit programming. This contract certifies that the producer agrees to all current MNN policies and protects MNN from any liability for airing the program.
